What to Bring When Visiting Our Hospital
- Please present one of the following documents at the reception:
- My Number Card
- Eligibility Certificate (for those do not have a My Number Card)
- Health Insurance Card (valid until December 1, 2025) - Recipient Certificate (for those eligible for public medical assistance)
- Referral Letter (if referred by another hospital or clinic)
- Medication Notebook
Use of the High-Cost Medical Expense Benefit System If you use your My Number Card as your health insurance card (Myna Insurance Card), you do not need to present a Maximum Copayment Certificate. Your copayment limit will be automatically applied with your consent at the time of reception.
If you have a My Number Card but have not yet registered it as a health insurance card, you can complete the registration:
- At our hospital using the card reader installed at reception
(Or online via your personal account on Myna Portal)
Once registered, your My Number Card can be used for insurance verification and to access benefits such as the High-Cost Medical Expense Benefit System.
Outpatient Visit Process
For patients who have a referral letter or do not have an appointment
- Please take a number from the queue ticket machine next to thecheck-in kiosk, and wait in the waiting area until your number is called.
- When your reception number appears on the monitor, please proceed toGeneral Reception Window No. 1 or No. 2.
- We will complete your outpatient registration and provide you with a reception slip.
- Please follow the schedule printed on your reception slip and proceed to the designated areas such as the outpatient consultation room, blood and urine test area, or radiology reception. Kindly present your reception slip at each reception desk.
For patients with a follow-up appointment
- Please insert your Patient ID card into the check-in kiosk.
- Please take the reception slip issued by the check-in kiosk and place it in the clear file.
- Please follow the schedule printed on your reception slip and proceed to the designated areas such as the outpatient consultation room, blood and urine test area, or radiology reception. Kindly present your reception slip at each reception desk.
Patients with a referral letter will be given priority for consultation.
*For patients visiting us without a referral letter from another medical institution, an additional fee (referred to as “SENTEI RYOYOHI”) will be charged in addition to the standard medical consultation fee.
- First visit
- Medical: ¥7,700 (tax included)
- Dental: ¥5,500 (tax included)
- Follow-up visit
- Medical: ¥3,300 (tax included)
- Dental: ¥2,090 (tax included)
Please present your health insurance card at the General Reception once a month at your first visit of the month.

About Payment
Please make your payment at either:
- The automatic payment machine located at the General Reception on the 1st floor, or
- The Cashier Counter at the same location.
